Other browser enhancements

Clock In/Out: Comments field is always shown on clock in and out forms.
Estimate - Items Tab: Grouping options are removed if they do not apply.
Company Drop-Down | All Browser Forms: Company Name and Company Code are concatenated if the Company Code exists.
Company Settings | Licenses and Addons: The Quantity column is now shown in the Addons grid.
Timecards that are in In Queue Status are cleared.
